What to Bring

New Patients

Patients who have never been seen at the Ochsner Cancer Institute previously are considered new patients. New patient appointments may be scheduled by calling 504-842-3910. Our specially-trained appointment coordinators can assist callers in selecting the physician whose training, interests and schedule best meet the patient's needs.

What should New Patients bring with them to their first appointment?

The following is a typical list of items that a patient should bring to their appointment. Providing these items at an appointment will help be to the patient's benefit, as it will allow us to expedite care. If a patient has questions about the items to bring, please contact your provider's office prior to your appointment. 

  • Insurance card
  • Co-payment
  • A picture ID card
  • A list of all medications that are currently being taken
  • Medical record (if not already in the MyOchsser, Ochsner Electronic Medical Record (EMR) system)
  • Copy of test results or x-rays
  • Pathology reports including slides, if available
  • Questions for your provider

Established Patients

Patients who have had at least one appointment at the Ochsner Cancer Institute are considered established patients. To schedule an appointment, patients should call 504-842-3910.
