Ochsner Home Health Corporation (OHHC), located in Kenner, Louisiana, was established in 1985, and currently services an average daily census of approximately 400 clients. Our agency was one of the first area homecare agencies to undergo JCAHO survey. In 2001, we received the outstanding rating of 99%, and a continued three-year accreditation.

OHHC provides state of the art medical care at home, which has the benefits of privacy in a relaxed environment, while at the same time allowing the medical services needed for recovery. Other benefits of home care include:

What is Home Care?

Home health care is the “provision of medical and/or rehabilitation services initiated by a physician for a patient in the home setting.” The purpose is to restore and maintain a patient’s maximum level of health, function, comfort, and independence. It is a collaborative effort of the patient, family, physician and other professional members of the interdisciplinary team. This health care team can consist of a skilled nurse (RN or LPN), physical, occupational or speech therapists; social workers and certified nursing assistants.

Who is Eligible?

The patient in need of homecare usually has multiple, complex medical problems that require increased assistance from others in addition to physician oversight. In order to be eligible for home care, patients must:

  • Have a medical necessity determined by his or her physician
  • Have a skilled need for nursing, physical therapy or speech therapy
  • Be confined to his or her home
  • Home care is covered by Medicare, Medicaid, private insurance, or can be paid for partially or completely out of pocket.

How is Home Health Arranged?

When your physician has determined that home health may be indicated a referral is called to OHHC. Depending on your diagnosis, past medical history, and physician’s concerns, a registered nurse or physical therapist is sent to your home within 24-48 hours to evaluate your home care needs.

Home care may be approved for reasons such as:

  • A decline in current medical status
  • Infusion therapy
  • Patient or caregiver education associated with diagnosis or new medications
  • Wound care
  • Onset of new disease or diagnosis
  • Physical rehabilitation
  • Speech or swallowing complications

When is Home Health Initiated?

Once a physician has determined a medical necessity, has made a referral to OHHC and it has been determined that you meet the eligibility criteria for home health by the admitting RN or Physical Therapist a plan of treatment is developed.

This plan is created with collaboration from the physician, the home health team and the patient/ family. This plan of treatment consists of interventions and goals that plot your course through home health.

All home health disciplines work very closely with the patient and family, the physician and each other to assist the patient to accomplish their goals and become as independent in their function and disease management as possible.
