State of the Industry

In 2021, U.S. healthcare spending reached $4.3 trillion, over 18% of the national GDP. With 10,000 people turning 65 each day, healthcare needs will continue to grow. Health systems and primary care physicians are facing dwindling margins amid rising labor, drug and supply costs. At the same time, clinicians are experiencing burnout at critical rates, with over 60% of physicians reporting burnout in 2021.

Daunting as it may seem, Ochsner Health Network (OHN) and Ochsner Accountable Care Network (OACN) are relentlessly focused on these issues, and value-based care (VBC) continues to stand out as the clear solution, driving down costs, providing enhanced clinician support, improving patient experiences and outcomes, and rewarding patient-focused care.

By 2030, the Centers for Medicaid & Medicare Services plans to enroll all beneficiaries in value-based care models, and it recently announced the Making Care Primary Model to accelerate VBC in primary care. This push toward patient-focused VBC models is echoed across commercial payers, and OHN is proud to help accelerate this momentous shift in healthcare.

Growth Through Collaboration

Backed by the breadth and experience of Ochsner Health, OHN launched in 2015 as a physician-led network of clinically integrated networks established to deliver high-quality care at a lower cost. Success in the value-based care space required initial investment in and development of supporting infrastructure, analytics, talent, leadership buy-in, and behind-the-scenes work with payers and employers to build an “engine” that could withstand industry pressures and reverse long-standing barriers.

In 2021, OHN restructured its business model, moving from a network of networks with decentralized governance, communications and capabilities, into a single, aligned network of engaged physicians and healthcare partners — a symbol of our shared commitment to improving our state’s health. Maintaining a patient- and physician-centered vision, we made further investments in our care delivery models, strengthened our population health and care management programs, and evolved our contracts and insurance products to demonstrate attainable value and realistic incentive structures for our clinicians.

Acceleration of Value-Based Care

Because addressing the needs of an expanding cohort of stakeholders requires innovation and agility, our strategy continues to evolve. Further investments in analytics, digital tools and education have helped us understand the broader determinants of health outcomes for our patient populations, leading to innovative ways of addressing those challenges. For example, through workflow optimizations and leveraging Ochsner Health’s tobacco cessation program, OACN achieved a performance score of 94% for tobacco use screening and cessation intervention in 2022. OACN also achieved top decile performance for future fall risk screenings and depression screenings and follow-up.

We’ve honed decision support tools, automated Epic prompts, and integrated analytics dashboards to help improve performance, patient outcomes, and experiences. For example, our outpatient care management program saw a 14% reduction in total cost of care for patients in their program, a total annual impact of $3.8 million.

Other notable impacts in 2022 include:

  • Diabetes testing was OHN’s highest performing quality measure in 2022, with 95% of patients with diabetes being tested.
  • Participating OHN physicians earned a $2.9 million distribution after saving employers and beneficiaries millions more through Blue Cross and Blue Shield of Louisiana’s Quality Blue gain share program.
  • OACN ranked in the top 5% of all ACOs for customer service and helped reduce Medicare patient care costs by more than $27 million.

Analytics-Driven Performance

OHN’s vision for the future is patient-centered, consumer-driven and physician-focused. We’ll continue to empower our physicians and care teams through resources like integrated care management teams and AI tools. More precise analytics tools will allow us to identify healthcare disparities in sociodemographic populations, and we’ll build partnerships to address inequities and social determinants of health. To meet a growing need, we intend to drive participation in newer Medicaid savings models.

At its core, value-based care is just really good healthcare. It’s the antidote to many of the industry’s prevailing challenges, improving efficiency and cost, and helping health systems and clinicians provide better, more collaborative care. With unwavering determination, we will continue to demystify VBC for our own stakeholders and the industry at large so we can make it more accessible to communities throughout the region.

An Ounce of Prevention

Providing preventive care has withstood the test of time, and its value is evidenced by better patient outcomes, improved satisfaction, and reduced costs. Through Ochsner’s Outpatient Care Management program, patients like Kimberly Shaney are enjoying improved well-being and quality of life through coordinated preventive efforts. Since her referral into the program for diabetes in 2022, Shaney has seen her A1C drop from 9.5 to 7.5, reflecting OPCM's overall success — 88% of enrolled patients with diabetes have their A1C under optimal control. Beyond diabetes, the program has helped Shaney stop smoking, receive physical therapy for fibromyalgia-related pain, install bathroom safety features, and access counseling and affordable housing.

OHN was awarded the United In Care Service Award in recognition of exemplary breast cancer screening performance, ranking in the top 10 Equity and Inclusion accountable care networks in 2022. The award was presented by the National Equity and Inclusion value-based care team and the American Cancer Society.
Providers participating in Blue Cross and Blue Shield of Louisiana’s Quality Blue contracts achieved a 90% or higher rating in multiple children’s categories. Those participating in Louisiana’s Managed Care Incentive Payment (MCIP) contract saw a 23% improvement in well child visits.
OACN achieved top decile performance in multiple preventive measures, seeing a 90% or higher performance rate for tobacco use screening and cessation intervention, future fall risk screenings, and depression screening and follow-up, with rates above 80% for most other measures.

Fostering Health Equity Through Technology

In 2022, Ochsner’s proprietary Diversity, Equity and Inclusion (DE&I) Dashboard launched, informing delivery model innovations and enabling clinicians to identify populations with gaps in care and outcomes. The dashboard includes patient experience and care metrics, with data sortable by sociodemographic groups like race, location, and insurance type. Through the dashboard, we could see that 36% of Ochsner Health Network patients with hypertension are Black or African American, and of those, 80% have their hypertension controlled. In comparison, 86% of white patients with hypertension have it controlled. This data was in turn used to inform strategies to close gaps in care and improve outcomes.

Ochsner Accountable Care Network

Accelerating Value in Rural Communities

Through its partnership with Ochsner Accountable Care Network, Titus Regional Medical Center in Mt. Pleasant, Texas, stands as a testament to the impact of value-based care on rural communities. Like many healthcare providers caring for underserved populations, Titus faces unique barriers like managing complex patients with limited resources. OACN’s scale of strategies — such as outpatient care management and evidence-based home health recertification reviews — have been instrumental in improving health outcomes and containing costs. Terry Scroggins, Titus CEO, says, “These efforts, coupled with quality and workflow enhancements, have helped us record remarkable changes in less than a year.”

Beau Raymond, MD, Executive Director of OACN, says, “Since joining the network in January 2022, Titus physicians, clinicians and administrative leaders have been highly engaged and actively partner with us to advance value-based care.” In less than two years, Titus has made significant strides in improving chronic disease outcomes, including a 9.5% increase in blood pressure control compared to the prior year. Additionally, less than 9% of the attributed patients had uncontrolled diabetes, which is 25% better than the mean ACO score for the measure.

Redesigning Care

The Ochsner 65 Plus care model is centered around individuals who are 65 and older, emphasizing more time with the care team and focusing on a holistic view of physical, social and mental wellness. Customized with the patients’ unique needs in mind, even the physical spaces are designed for safety and comfort, with features like nonslip flooring and accessible entries. Since many Ochsner 65 Plus patients are covered under a value-based contract, OHN collaborates with those organizations to capture analytics and disseminate insights that help enhance care for our seniors.

Financial Performance

Shared savings are earned by reducing healthcare costs while prioritizing efficiency and high-quality care. Care management fees are earned for exceptional quality and patient outcomes. For patients, this means the right care delivered at the right place and at the right time. For clinicians, earned shared savings and care management fees are a testament to their dedication and commitment to patient-centered care. For employers and payers, the significant savings and preferred outcomes highlight the profound impact of a value-centric healthcare partner.
