Visit
The receptionist will ask you to verify information such as your address, date of birth and contact telephone numbers so that your records will be current. He/she will also ask to see your photo ID and insurance card each time you visit our facility so that we can ensure your visit is billed to the correct insurance carrier. New patients should arrive 15 minutes prior to your scheduled appointment so that we can enter your insurance information in the computer and you can complete a history form for your visit.
