Performance Improvement Coordination

OHN's Performance Improvement team works with partner providers to review processes, identify opportunities for improvement and deploy initiatives that support continuous progress within the framework of payor‑defined quality and cost metrics. The PI team supports our partner physicians in many ways, including:

  • Relationship-Building: Physician practices are assigned dedicated PI Coordinators who analyze existing workflows and develop meaningful relationships with providers and other practice personnel.
  • Education: PI Coordinators foster effective physician‑patient interactions — providing care opportunity reports ahead of patient visits to ensure any chronic conditions are captured accurately during visits. They provide education on how contracts work and why they are important. Providers complete Ochsner Learning Network modules to educate on common goals across the network, like appropriate ED usage, clinical integration and readmission. All providers have scorecards that track performance on quality measures across contracts. PI Coordinators work with providers on performance and opportunities for various payor benefits that are available to members, e.g., mail order pharmacy benefits, silver sneakers, diabetes educators, smoking cessation, etc.
  • Flexibility: PI Coordinators meet providers where they are in their progress. Two‑fold flexibility in both locations and times for meetings that accommodate provider schedules, as well as adjusting for varying levels of engagement by working with providers to optimize patient care and manage chronic conditions.
  • Incentives: Community PCPs are incentivized and receive quarterly reward bonuses, and primary care providers and pediatrician members receive annual distributions based on overall annual contract performance.
