Laura first heard about Digital Medicine during a routine doctor visit in 2017. After trying to manage her hypertension (high blood pressure) and type 2 diabetes for over 10 years, she found out that her systolic blood pressure had reached 200 and her A1C was hovering at 8 percent.
After that visit, Laura’s primary care doctor convinced her to enroll in Digital Medicine. As part of the program, Laura was set up with a digital blood pressure cuff and glucometer and paired with a dedicated care team comprised of a pharmacist and personal health coach. Using her new digital devices, Laura was able to take readings from home where they were automatically uploaded to her electronic medical record for her program care team to review and adjust her medications or discuss healthier habits, if needed.
Coming from a big family who loves family dinners and cookouts, Laura never really thought about what she was eating or how it was affecting her health. Working with her health coach, Laura gradually began trying out healthier food options and walking at her local park near her home in Belle Chase after work. Since then, Laura has lost over 50 pounds and has seen her systolic blood pressure drop down to 119. She also has more energy which she can use to spend time with her grandchildren.
