Am I eligible to join?

To join Digital Medicine, you need to be at least 18 years old, own a smartphone, have a diagnosis of high blood pressure and/or Type 2 diabetes.*

What will it cost?

Ochsner Digital Medicine is a covered service under many employer benefits and health insurance plans (which may be subject to applicable patient responsibility amounts).

Does this replace my regular primary care provider?

Ochsner Digital Medicine supports you in-between doctor visits to help you manage your condition as effectively as possible. The Digital Medicine clinical team monitors your readings to ensure you're on the best medication, and your health coach will provide support on building healthy lifestyle habits.

You will still need to visit your regular provider for all other care.

Is someone monitoring me 24/7?

While we are not a 24/7 monitoring service, your Digital Medicine care team will regularly review your readings and reach out to you if you are starting to trend off course. They will call to help you manage your blood pressure and/or Type 2 diabetes over the phone. Your clinician can adjust your medications, and your health coach will help you build healthy habits.

What happens after I begin the enrollment process?

We’ll ship your device to your home and send you the tracking number via MyOchsner. Once you have your device, follow the instructions included in the shipment and take your first reading. Once your reading is received, you’ll get a call from our team to finalize your enrollment, and then you’ll meet your dedicated care team.

I’m not very “tech-savvy,” is the program still right for me?

Absolutely! Your smartphone makes it easy to connect your digital device and submit readings to your care team. If you have any issues, our dedicated support team will help you every step of the way. You’ll also have access to live chat support, technical support appointments, how-to videos and set up guides.

How can I get my device?

Your device ships directly to your home, or you can pick up your device at one of our O Bar locations. View the list of O Bar locations here.

How often should I take a reading?

After you first join the program, it’s best to taking readings as often as possible so we can build a baseline to customize your care. Once you’ve established your baseline, the program asks that you take one blood pressure reading per week for participants in the High Blood Pressure program.For participants in the Diabetes program, you and your clinician will work together to determine a blood sugar testing schedule that works for you.

What is the care team and what will they do?

Each patient in Digital Medicine is matched with a dedicated care team made up of a Health Coach and Clinician. The Health Coach helps you build healthy lifestyle habits, and the Clinician monitors your readings and adjusts medications as needed.

What kind of credentials does my care team have?

Your care team consists of a Clinician and a Health Coach. Our Clinicians are certified Physician Assistants (PA) or Clinical Pharmacists (Pharm-D); Health Coaches are highly trained in various allied health fields including nutrition, public health, social work, health education or kinesiology. They also have specialized skills to help you make healthy changes.

How will I communicate with my care team?

Your care team will monitor all the readings you submit as part of the program and will communicate with you via phone and through MyOchsner messages.

How do I send my readings to the care team?

We’ll send you a blood pressure cuff and/or glucometer that you’ll use with your smartphone to submit readings digitally to your care team.

Will my doctor be able to see my progress?

All of your digital medicine progress is recorded in your medical record and available to your doctor.
