In 2016, our Telehealth Network saw over 200,000 patients virtually.
CareConnect 360 allows partner hospitals to access specialists fast when treating patients with critical to to complex conditions -- and to treat them right on site. To date, more than 79 percent of patients evaluated through Ochsner's Telehealth program were able to remain at a local hospital for their care.
The network is based on a hub and spoke model which encompasses our promise to keep local patients local. If a patient needs a higher level of care, they are recommended to transport to the designated regional hub or Ochsner main campus.
Our spoke sites are never held to a specific transferring facility. If a partner had a pre-existing relationship with another facility, the Care Connect 360 program does not disunite that relationship.
Each partnering site has an Ochsner appointed RN Clinical Coordinator that works with the spoke site to improve national benchmarks in Telehealth care to the patient. This encompassing approach proves best clinical value to both the spoke site and the Ochsner system.
Each year, Ochsner host a CE accredited Users Group that brings the entire Telehealth Network of facility's together to collaborate on best practices to strengthen patient care.
