Connected MOM (Maternity Online Monitoring) uses digital tools to provide expectant mothers a convenient way to manage pregnancy from home.

Once invited to enroll by an obstetrician, patients are given a kit containing a digital blood pressure cuff, a digital scale and educational materials. The digital blood pressure cuff and scale connect through the patient’s smartphone to automatically transfer all home readings to their provider team in real-time. Throughout pregnancy, patients take readings with the devices, and at three scheduled times, they are prompted to submit the results of a urine protein test in their patient portal. If any readings or lab values are out of range, the provider team is notified and can proactively respond. Moreover, by collecting this data digitally, patients can skip up to three regularly scheduled pregnancy visits, making care more convenient and opening capacity in the clinics.
