Our Process

We believe in partnership to achieve better health together. We communicate with you every step of the way as your patient progresses through our healthcare system.

Referral Received:
The journey begins when you submit the referral, connecting your patient with Ochsner’s trusted care teams.

Awaiting Scheduling:
In this stage, Ochsner ensures every detail is in place for a smooth transition, whether gathering additional information from the patient — such as insurance or demographics — or coordinating with the department to finalize scheduling.

Scheduled:
Once all details are confirmed, we notify you and the your patient of the scheduled appointment time.

Seen:
The patient completes their appointment with Ochsner’s expert care team.

Follow-up Scheduled:
We promptly schedule any needed follow-up appointments to maintain continuity of care and ensure your patient’s health journey remains on track.

To better serve you, please have the following information: Referring Physician Information including name, address, phone number, fax number

Patient Information

including:

  • Patient demographics: name, sex, date of birth, address, phone number
  • Insurance information: all active primary and secondary insurance coverage
  • Requested specialty or program and if a specific doctor is requested
  • Most recent clinic note(s): multiple notes may be included, depending on the case
  • Imaging results and lab work, if applicable 
    Radiology orders:  a signed order with the CPT code(s) and ICD-10 code(s)

At Ochsner, we accept most major insurances but advise patients to confirm coverage details directly with their insurer.

What Happens Next?

Our team will work diligently through concierge services to ensure that your referral is handled in a timely manner.

You can feel confident in your patient's care, knowing that we implement physician orders during all transports, including those for specialty ventilator, isolette and balloon pump patients, and if needed our physicians and specialty staff are available to accompany your patient during transport.
