  • Using Bluetooth enabled devices, patients submit regular readings from their smartphone
  • Readings flow directly to the Digital Medicine team and trigger proactive outreach and intervention by a dedicated care team of clinicians and health coaches

• Who is Eligible?

  • Existing diagnosis of high blood pressure or Type 2 diabetes
  • Must possess a smartphone (Android or iOS)
  • Not subject to clinical exclusionary criteria

• What are the Benefits?

  • Digital Medicine Clinicians continually monitor the patient’s connected device readings and are able to intervene and adjust the patient’s current medications, if needed
  • Together with the patient, the Digital Medicine Care Team will develop a plan that includes personalized recommendations to help patients improve their health
  • The Digital Medicine Care Team will outreach to the patient regularly to check in and help with their health goals
  • Patients and their physicians will receive regular progress updates with reading trends

  Digital Medicine’s high blood pressure program and blood pressure cuff are available at no cost to you.

  Digital Medicine’s Type 2 diabetes program, glucometer, and supplies are available at no cost to you.
